The Process For Your In-Home Sleep Test:

  • Your medical or dental provider sends your referral to us.
  • You will be contacted within 48 hours.
  • Your insurance benefits and out-of-pocket costs, if any will be explained.
  • Schedule your in-home sleep test with direct shipping to you.
  • After your device has been returned, your results will be reviewed by a board-certified sleep physician. Diagnosis and treatment recommendations will be provided.
  • The results will be sent to your ordering provider within 2 business days.
  • Contact your ordering provider to go over results.

Scheduled Return of Sleep Equipment

  • We ask that you track your package via the email sent to you (please check your spam folder). Your first night of testing should be completed on the day your device is delivered to you.
  • The device needs to be returned to us on the 3rd day. If you arranged to wear the device on specific days then your device is to be returned on the day agreed upon.
  • If you are unable to test once you receive the testing equipment, it will be your responsibility to contact Complete Sleep Relief to arrange for dates that you are able to test and return the equipment.
  • Failure to communicate will result in late fees charged at a daily rate of $45.00 (forty-five dollars). If the device is more than 10 days past due, you are responsible for the cost of the equipment. That cost is $1,500.00 (one thousand five hundred dollars). You are not responsible for delays caused by the shipping service.
